HOPE Dementia Support-a program of CDM Caregiving Services has been awarded a Community-Clinic Integration Grant from Kaiser Permanente Northwest. 

HOPE’s mission is to provide support, education and advocacy for care partners of loved ones living with dementia. With Unite Us, HOPE now has the tools and resources to navigate social, medical and eligibility services on behalf of each individual. The Unite Us platform also allows the Unite Washington network to refer care partners of loved ones living with dementia to the HOPE dementia support groups.
